I am providing the above information out of my own free will and I gave consent to enrol myself as Direct Seller with Altos Enterprises Limited and the Sponsor has agreed to use her/his Distributor Id for the same.

I am aware that Altos Enterprises offers no job opportunity to me and I also understand that it has a quality product portfolio which is sold and promoted through its Distributors. I also understand that Company does not provide any investment opportunity or offers any fixed income.

I confirm that I am aware about the business opportunities offered by Altos Enterprises Limited where I can purchase Altos products either for self-consumption or resale.

I acknowledge that there is no fees for joining Altos Enterprises Limited nor any fees for giving training and I have not paid any sum to anyone including company to facilitate my joining, training etc. as Direct Seller.

I have not been misled, coerced or influenced to join Altos Enterprises and I have thoroughly understood its Terms and Conditions and its Business Opportunity Plan before joining the Company as an Direct Seller.

I understand that my agreement as an Direct Seller of Altos Enterprises Limited is on a Principal to Principal (P2P) basis.

I have been provided with free Orientation Program by my sponsor who has provided fair and accurate information on all aspects of Company's Direct Selling Operations, about free joining, it Sales Incentive System, policies of Return and Refund within 30 days, Return to Origin, expected incentives and related rights and obligations as governed under the policies of Company read with Direct Selling Guidelines issued by Ministry of Consumer Affairs, Food & Public Distribution.

I have understood the Terms and Conditions mentioned in the Direct Seller Application & Agreement Form including the constituent documents as amended from time to time.

I further understand that this application and any ensuing Company Distributorship upon acceptance of this application by Company shall be subject to the Terms and Conditions given below including the constituent documents as amended from time to time.

I confirm and undertake to keep my all necessary details updated with the Company in case of any subsequent change in the future.

I understand and agree that information provided by me in Direct Seller Application and Agreement Form shall be used in accordance with applicable laws in India.

I have read and understood the Guidelines and Code of Ethics of Altos Enterprises Limited and shall abide by them.

I am signing Direct Seller Application and Agreement Form out of my own free will and all the information provided by me are true and correct and nothing has been concealed therein.

I hereby accord my consent to Altos Enterprises Limited for conducting KYC for authentication of my documents.

Any violation of the Terms and Conditions by me shall result in disciplinary actions by Altos Enterprises Limited.

I confirm that I am registered under the GST rules and regulations and my GSTIN is ______________________________I will raise a monthly invoice with GST at the applicable rates on Altos Enterprises Limited. I further confirm that I will deposit GST amount timely and in such a manner that Altos Enterprises Limited is able to claim input credit of GST amount, if available.

OR I am not registered under the GST Act as my turnover is below the prescribed limit. I further confirm that I will immediately obtain GST registration and inform Altos Enterprises Limited if my turnover crosses the GST exemption limit.

I hereby confirm that I have been personally explained by my sponsor about the company's Business as per its Sales Incentive Plan. I also agree that if no sales of products are made by me for a period of up to one year after signing this Agreement then Altos Enterprises Limited has the right to terminate this Agreement in accordance with the Section 10 of the Indian Contract Act, 1872.

I hereby also confirm that I have been personally explained by my sponsor that there is 30 days cooling off period after signing Agreement & Application Form which entitles me to return any product purchased and seek refunds during this period after deducting taxes & given commission . I understand and I am aware that after the expiry of 30 days i.e. after expiry of cooling off period I will not seek any kind of refunds or return of any purchased products.

I have understood all the terms & conditions and hereby confirm to become Altos Direct Seller.

Distributorship with the Company shall be subject to receipt of complete Documents and successful verification. The Company reserves the right to reject the application for any reason and to terminate or revoke distributorship for any reasons including but not limited to provision of incomplete, inaccurate, false or misleading information.

The Applicant must sign each page of the Direct Seller's Agreement Form & upload in profile or mail us at info@altosindia.net with self-attested copy of Pan Card, Voter Card, Cancelled Cheque or Bank Passbook.

Distributorship with the Company shall be subject to receipt of complete Documents and successful verification. The Company reserves the right to reject the application for any reason and to terminate or revoke distributorship for any reasons including but not limited to provision of incomplete, inaccurate, false or misleading information.

By signing up, I expressly authorize the Company, which will override my DND registration, to send communications on my mobile.

By filling this Application Form & Signing this Agreement, I hereby agree to become Direct Seller of Altos Enterprises Limited, subject to the consent of Altos Enterprises.

I confirm that I have read the Terms & Conditions of this Agreement and I undertake to abide them without any reservation.
